Start with the construct
Perceived stress, acute reactivity, anxiety, fatigue, sleep disturbance and physiological arousal overlap, but they are not interchangeable. The first step is to define the intended effect and the time frame in which change is expected.
A questionnaire about stress over the past month answers a different question from cortisol sampled during a laboratory challenge. Adding more measures does not automatically improve a study if their purpose is unclear.
Patient-reported outcomes
Validated questionnaires are often the most direct way to assess how stressful or manageable life feels to participants. They are practical, scalable and clinically interpretable when the measure fits the target population and protocol.
Trials should identify a primary outcome, avoid excessive duplication and consider response burden. Repeated brief ratings can help characterise timing and day-to-day variation but require a clear analysis plan.
Physiological and behavioural measures
Biomarkers can complement self-report, but each has important collection and interpretation constraints.
- Cortisol: sensitive to time of day, awakening, food, exercise, medication and sampling adherence
- Heart-rate variability: influenced by breathing, posture, recent activity, recording length and device quality
- Blood pressure and heart rate: useful for acute reactivity when measurement is standardised
- Inflammatory or metabolic markers: potentially relevant, but generally non-specific to stress
- Wearables: valuable for repeated real-world data, although proprietary algorithms and missing data require attention
- Performance tasks: can assess attention or control under challenge, but practice effects and task validity matter
Measure reactivity and recovery
A standardised laboratory stressor can test how participants respond to challenge and how quickly they recover. This may be more informative than a single resting sample, particularly when the proposed intervention is expected to influence acute resilience.
Sampling times, pre-visit restrictions, environmental conditions and analysis methods should be prespecified. The protocol should also state whether a measure is confirmatory, secondary or exploratory.
Triangulate without overclaiming
Convergence across a validated questionnaire, an acute challenge and a physiological measure can strengthen interpretation. Disagreement is also informative: people may feel better without a detectable biomarker change, or physiology may change without a meaningful improvement in daily life.
The best measurement strategy is not the largest battery. It is the smallest coherent set capable of answering the study’s primary question credibly.
Define the aspect of stress first, then select measures whose timing, context and interpretation genuinely match that construct.
